Readmit Rescue™ - Hospital readmissions are often unavoidable consequence of life-threatening complications that can appear after discharge.  However, research reveals that approximately 50% of all readmissions are preventable.  During transitions, patients with complex medical needs, primarily older patients, are at risk for poor outcomes due to medication errors and other errors of communication among the involved healthcare providers and between providers and patients/family caregivers.  Our specialty trained Registered Nurses will work with the client and all those involved in their care to ensure anticipated outcomes.  Readmit Rescue has redefined post hospitalization care, significantly improving patient outcomes and satisfaction.

DOVES' specialty programs are designed to enhance the lives and address the unique needs of our clients.

 

AidChecks™ - Sometimes a friendly connection is all that is needed to ensure compliance and peace of mind.  For a nominal fee, we call our clients daily, or more frequently as requested, to confirm client safety and that needs are being met.  Our compassionate care aids offer medication and appointment reminders, answer general care questions, inquire about physical health and mental well-being, and establish transportation as needed.  One of the biggest challenges for seniors is taking the right prescriptions, dosages, and frequency, especially since they often receive prescriptions over time from several different physicians - AidChecks works to alleviate this issue.

"Bed to Breakfast" and "Tuck-in" Programs - Mornings and evenings are often the most challenging times for seniors. Mornings are commonly accompanied by stiffness from a long period of inactivity. Similarly, evenings can be difficult after a long taxing day. DOVES recognizes that many individuals only need assistance in the morning or at evenings, and therefore does not require a four-hour minimum. Our certified nurse assistants and caregivers are available to support and encourage individuals with getting in and out of bed, stretching, dressing, bathing, toileting, meals and medications.  

Transportation - For a nominal fee, Doves clients may request wheel-chair assist transportation to and from:

 

  • doctor appointments,

  • pharmacy,

  • special occasions,

  • friends and family,

  • shopping,

 

We do not simply pick you up and drop you off at your destination - we will help you get ready and accompany you to your appointment. Call for details and rates.
